About the ECM Program
Our Enhanced Care Management (ECM) program delivers a whole-person, interdisciplinary approach to care, addressing the clinical and non-clinical needs of clients with complex medical and social conditions. The program focuses on:

The ultimate goal is to improve care coordination, address social determinants of health (SDOH), and reduce unnecessary healthcare utilization for high-need Medi-Cal beneficiaries.
Job Overview
Reporting to the Program Manager, the Case Manager delivers high-quality, client-centered case management services for individuals and families experiencing homelessness. You will manage a caseload of 15–20 participants, serving as their central point of contact.
Key Responsibilities
Conduct initial and quarterly psychosocial assessments
Collaborate with participants on individualized service plans and housing stability goals
Build trusted, professional relationships with clients
Provide outreach, housing navigation, and ongoing support
Facilitate referrals to healthcare, mental health, substance use, and community services
Support clients in obtaining employment, benefits, and education opportunities
Accompany clients to key appointments as needed
Use a harm reduction, strength-based, and client-focused approach
